Reporting an issue is always free
Anyone can scan a QR poster and file an incident without an account or a licence — a contractor, a night-shift operator, somebody walking past a spill. You pay for the people who manage quality, never for the people who report a problem. Charging per reporter is how quality systems end up with nobody reporting anything.
